Transitions: A Counseling Center for Individual and Family Therapy, LLC is a Family Therapy Center located in Glastonbury, CT (Hartford County). Conveniently located near Hebron, Manchester, Marlborough, Middletown, Portland, Rocky Hill, and Wethersfield, please see our directions below.

Payment
Our out-of-pocket fees are:*
$160 for an Initial Assessment visit - 1 hour including paperwork
$145 for subsequent sessions - 60 minutes
$125 for subsequent sessions/couples/family - 45 minutes
$250 for 90-minute Imago Couples Therapy Sessions (with Carey)
$65 for each Group Therapy Session (vary from one hour+)

*You will likely NOT be responsible for the full fees listed above if you use insurance that we accept.
Fees, co-payments or estimated co-insurance must be paid at the time of service using a card and our IVY Pay App Service. You don't need to download the App; we will help you set up payments during the first session.
Please note that we may need to temporarily suspend counseling sessions if you are not up-to-date with paying your balance. The reason for this is to preserve our time taking care of your treatment needs rather than to follow up with you to obtain payment.

*Please note that if you have Medicare, we are not able to bill them, as we are not on their list (LMFT's and LPC's are ineligible to receive Medicare payments). As a result, we also cannot process payments for your secondary plan if you have Medicare, unless Husky is your only other plan. We would need to have you pay us and then we could provide the necessary information for you to file with your insurance plan(s) in that case. We apologize for any inconvenience!

Directions/Getting Started:
From Rt 2, take the Hebron Avenue Exit (#8). If coming from the East, take a right and then another right off of the exit onto Hebron Avenue. Eastern Blvd is a left at the second light. You will see "Gemma" on the corner. We are in the 3rd building (tan colored) on the right-hand side of the road.
If coming from the West (Hartford area), take a left at the end of the exit onto Hebron Avenue and then Eastern Boulevard will be at the third light.
All you need to do is bring your insurance card, a photo id, and your specialist co-pay, co-insurance, and/or deductible payment. We accept cash, check or credit/debit cards besides American Express. There is a quick and easy form to fill out to make a payment by card or to place a card on file with us. There is a small amount of paperwork to start.
